I understand that my puppy will need to receive an additional DHPP/DAPP vaccine at 13 and 16 weeks before being fully protected against distemper, parvovirus, adenovirus, and parainfluenza.
I understand that my puppy is due to receive a rabies vaccination at 16 weeks of age, and that this vaccine is required by the state of Michigan.
I understand that my puppy has been microchipped, and to switch the puppy to my name I need to register the microchip with the Avid and pay the one time registration fee with the included registration paperwork.
I understand that my puppy is sold as a PET ONLY. I agree to not breed this dog, and to spay/neuter by 8 months of age. Sweetie Doodles recommends spaying/neutering at 4-5 months old, before the first heat cycle in females.
I agree that if for any reason the dog needs to be re-homed, the dog will be returned to Sweetie Doodles.
